One aspect you mentioned of the epoxy bolts concerned overhead 
installations.  ITW manufactures a plastic cap that wedges into the hole. 
 The cap has a slit in it which allows insertion of the nozzle of the 
injection tube into the hole and prevents the epoxy from running out.  It 
also provides a sufficient wedging action that will hold the anchor in place 
while the epoxy cures.

The cap is referred to as a "hole plug" in the ITW literature.
