There is a video called ENGINEERING (?) DISASTERs made by PBS (?)   
covering Hyatt and other spectacular disaster . It was on TLC ( Learning   
Channel ) few months ago. You may contact TLC to get the video.

There are two very good books that cover this:
 -Why buildings fall down- by Mario Salvadori, and
 -The role of human failure in design engineering, by Henry Petroski.
