You can check out "Designing Floor Slabs on Grade" by Boyd C. Ringo & 
Robert B. Anderson which you can order through ACI. It has numerous 
charts for design of vehicle axle and wheel loads.

I also believe that "Fundamentals of Reinforced Concrete Design" by 
Ferguson, Breen and Jirsa has a chapter on special loads with good rules 
of thumb for concentrated loads on slabs (if your slab is floating.)
