Forgot to add that you may want to check the displacement at the top of the pier
to see if the displacement at the maximum load case is acceptable for your
design.  It is not unusual to see top of pier horizontal displacement of 1 to 1.5
inches.  There may also be significant angular displacement to consider depending
upon how you connect to the pier and the rigidity of the final design.

Displacement and rotation can be determined with a solution of the pier as a
Winkler beam.  (Tedious) or by solving it with a finite difference solution
published by Reese and Wang and used in LPile. Sorry don't have the publish date.
