 I have used the following mix on many occasions for fill:
 1 1/2" rock		 800 pounds
    3/4" rock		1000 pounds
 sand			1500 pounds
 cement			  141 pounds
 water			    30 gallons
This looks like a receipe for a Swiss watch (made of concrete, of course [ 8^)
We use a much simpler specification: 
	"5 sack mix" is 5 sacks of concrete per cy and gives a 3000 psi concrete
	"4 sack mix" gives 2000 psi concrete
and a "slurry" for a flowable backfill stronger and more compact than any soil
and yet can be dug up with soil digging tools is spec'd as a "2 sack mix".
These are obviously good ol' boy rules of thumb not meant for critical
applications, but they have worked for  a long time, and all the ready mix
suppliers (who all seem to be good ol' boys) will certainly know what you are
talking about.
