The basic tensile stress limit is 6SQRTf'c.  The 12SQRTf'c is only allowed
under certain conditions that have to be satisfied.

I believe, that most of the cracks in post tensioned slabs are caused by creep
and shrinkage, locked in stresses at wall and/or beam corners, pour strips
that were closed too early!!
