Could it be that the problem was not overtightening but rather a bolt that
did not conform to the standards.  You may have been lucky in that  they
were still able to function if installed with care and thus you could allow
the Contractor to continue to use them.

If the bolts were used in such a way that additional tension sould be
applied to them in use, you might not want to allow the use of these bolts.
