Nice to see someone with the guts to list an engineering error amongst 
all the contractor and architect miscues. Probably what happened to the 
column design is a result of the major flaw in most of the checking jobs 
I've been part of. The checker only goes through the arithmetic and 
ignores the design premises (in that case, end fixity). Seen it many 
times when checking is assigned to a junior person, who usually assumes 
that the designer, being more experienced, wouldn't make a mistake 'like 
that.'
