The problem was in cross-grain failure of the plate as the wall tried to 
uplift from the foundaton by the connection of the panel nailing to the side 
of the sill. This created the typical problem that we associate with ledger 
connections however, an increase in bolts with larger washers lowers the 
pressure at the bolt so that a 2x plate is protected from cross-grain failure.
