Dear Jimmy:

There was supposed to be inspection of the construction (not of the design) by an independent testing and inspection agency.  It was the responsibility of the contractor to notify this agency when inspection was necessary at specific phases of the construction.  However, as it turned out, the contractor never contacted the agency once the foundation and floor slab work had been completed.  An expensive oversight!
