Charlie,

Thanks for your input.

I agree with your EOR comment.  However, for this project the Contractor
(and therefore the mast arm fabricator) was responsible for the design of
the structure and foundation, and therefore the fabricator is the "EOR" for
the structure.  This is a common practice for mast arm structures for
non-DOT work in my area.
