guide. That indicates a "rush to press". <<<

That's right.  ACI 318-99 was supposed to include an appendix that addressed both cast-in-place anchors and post-installed anchors.  But the post-installed section was not ready in time to meet the IBC 2000 deadline.  Thus, PCA published the cast-in-place anchorage information separately and there are many errata.  They should get most of the wrinkles ironed out for ACI 318-02.
