Might want to get ahold of PCA's "Strength Design of Anchorage to Concrete".
Its probably more uptodate than Gaylord's book.

Unfortunately, no one in this office has a Structural Engineering Handbook.
Would anyone be willing to email me the appropriate formulas for calculating
anchor bolt embedment depth and anchor bolt uplift force.  Again the
application was for a base ring supporting a 42 foot tower.
