I don't see any reason to add this to your notes.  I see adding the note as
taking liability for the integrity of some sheathing panel that has been
exposed to moisture and possibly (probably?) abused.  I think your
contractor has had an inspector question the use of forming panels for roof
sheathing, and wants evidence that "The engineer says it's OK" when he gets
called on it.

What would APA have to say about this?  If they would approve (since they
are the ones rating the panels), I might go along with it, but I don't think
I would put it in writing.  If they're approving it, they should put it in
writing, not you.

That's my take on it.
