Perhaps the most important item to note in this regard is that the third edition of the PTI "Design and Construction of Post-Tensioned Slabs on Ground" manual is coming out "any day now." It will have a greatly enhanced (read "different") methodology based on extensive research and the development of a model for these foundations based on an "elastic half space" treatment.
 
Believe it or not, a LOT of thinking and (especially) arguing has gone into this publication. This is a "hot topic" here in the Southwest, and seemingly every structural and geotechnical engineer has his/her iron in the fire.
