Sounds like the problem right there.
 
The design should be, IMO, to provide sufficient reinforcement per "subgrade drag", "WRI" or similar method, to ensure that normal expansion can be handled. The slab may simply be behaving as designed--which is a bad thing, apparently.
 
Repair may be difficult. I'd have to think on it a bit.
