I would not say it is common, but I have seen some waffle slabs being constructed.  Recent projects (last ten years) that I know of that used a waffle slab design are one chip manufacturing facility, one airport and two libraries.  The peak of popularity may have passed but they still have distinct advantages in some applications, particularly very heavy live loads.
All that being said, I have never designed a waffle slab on one of our projects.

Out of curiosity, how common is waffle slab construction, in the US and elsewhere at this point?
Over the years, I've been asked to look at several waffle slabs in the Northeast and the mid-Atlantic area.  All of there were built sometime in the 70's.  That seems to have been the peak of its popularity as an architectural concept.
Is it used more in other parts of the country?
