Try the FEMA website for Urban Search & Rescue. There are standard forms they use for retaining engineers.

I am searching for contract documents/templates for the "temporary, or permanent" employment/retainment of engineering professionals, by local authorities, for (engineering) response to emergency events. 
 
Does anyone have experience with, and/or access to such documentation?
