I have heard many "horror stories." I can't imagine having a bureaucrat
with "life and death" power over whether or not my relative will have
treatment or have to wait (or even be denied).

Here there are always alternatives. 

I have a good friend, a young twenty-something woman whose child was
born with a severe heart malformation. These are just kids; they
probably make about $30,000 a year together, if that.

The kid got a heart transplant. He received all the subsequent care.
Some of it was paid by Medicaid, some by hospital "grants," some by
charitable contributions. The family did not have to sit and hear a
bureaucrat say "sorry, your child doesn't fit the profile."
