Place the concrete with the opening boxed out. Make the fill side opening 4" larger than the basement side, with a "step" about half way in.  Make a second placement for the knockout, placing a bentonite water stop around the perimeter of the opening.  If the opening is large enough, you may need to reinforce the knockout and/or thicken the wall around the knockout area in order to get the shear resistance at the "step" in the wall, where the effective wall thickness is only 1/2).
