I don't know about the large-gap products from Hilti, but most adhesives (epoxies included) have fairly specific bond thicknesses for maximum performance.  Although there would be little empirical data to go from, would it be possible to drill & tap a 2"dia or 3" dia rod to receive threaded rod for your anchor bars, and then epoxy (Hilti HY-150 or similar) your new "inserts" into the oversized holes?

I have a project where the anchor bolts were not installed correctly.  The contractor wants to core 2-3” diameter holes for installing new bolts.  I was wondering if there was a product that would bond to the bolts and a hole that big.  The anchor diameters are ¾” & 1”.
