A point of clarification:  I'm not trying to repair the damage that I observed.  That is NOT my project.

Rather, I am trying to be pro-active in trying to prevent similar problems on a NEW facility that I am designing.

You should not make any assumptions about the source of the deterioration or make recommendations for repairs until you have cores taken for petrographic analysis, otherwise you're just flying blind.
