I remember doing some research for something similar, and I found an USACE or NAVFAC doc that mentioned something like 4" clear cover for piers.  Check the UFC (Unified Facilities Criteria).

I am aware of some discussion in some ACI guides and commentaries and some requirements in European standards, but I am looking for "mandatory language" documents in the U.S.
