I think what you are referring to is a drop in anchor (e.g. Simpson Drop In Anchor with expired ICC ESR 1396). I've had a difficult time finding one which has a current ICC report number.

Anyone out there know of a post-installed concrete insert that has internal threads that would allow for an all-thread rod to be threaded in?

Similar to a ferrule or burke insert only it's drilled in to existing concrete?
