The last sentence tells the real problem.

Kane added, “After seeing everything else that has happened, it seems like 
 more of the same. It shows kind of a wanton disregard for good engineering
design and practice.”

When you pay an engineer to design a concrete airplane - Don't blame the
mechanic or pilot when it doesn't fly.
